Standard choice form
If you're eligible to choose a fund, your employer will give you a Standard choice form.
The form sets out your options for choosing a superannuation fund.
Your employer will fill out Section B, which tells you what fund your employer's Superannuation Guarantee contributions will be paid into if you don't choose a fund.
Section A sets out the information you must provide to your employer if you want to choose a fund.
