Standard choice form
This is a WA State Government form that is given to employees so they can choose a fund. It includes details of the default fund (called the 'employer fund') to which you will contribute if an employee does not choose a fund.
What to include on the Standard choice form
You are only required to complete 'Section B: Employer to complete' of the Standard choice form.

When to provide the form to your employees
You must:

Provide a Standard choice form to all existing eligible employees – within 28 days of the introduction of choice

Provide one to new eligible employees within 28 days of their start date.
You only have to provide a Standard choice form to a new employee when they first commence employment. This means you are not required to provide the form to the same employee if they commence employment again (such as a casual or itinerant worker who you employ in busy periods or at the same time each year).
However, you will have to provide another Standard choice form to your eligible employees:

Within 28 days of an employee requesting a form in writing.

Within 28 days of becoming aware that you are unable to contribute to an employee's chosen fund or that it is no longer a complying fund.
There is no limit to the number of times an employee can choose a superannuation fund.
You can provide a Standard choice form to an employee electronically – for example, by email.

If a fund has already been chosen
If an eligible employee chooses a fund before you give them a Standard choice form, – say, on their first day of work - and they give you all the necessary information, you are required to act on their choice. You do not need to give them a Standard choice form.
If your employee leaves
If an employee ceases to work for you during the period when you are required to provide a Standard choice form, you need not provide one.
